Package for ReCODE and PreCODE Participants.

This program includes:

• Twelve (12) sessions with this health coach (Session: 45 minutes).
• Development of a personalized plan based upon your PreCODE or ReCODE Report or your lab tests, and health guidance with lifestyle strategies which include addressing preventive or root causes of cognitive decline based upon the protocol and your strengths, challenges, abilities, experience, agreement on goals, etc.
• Cognitive assessments to track progress (ReCODE participants: SLUMS or MoCA, the AQ21, and CNS Vital Signs and for PreCODE participants: SLUMS or MoCA and the Cq Assessment)
• Two PreCODE or ReCODE Reports to track your progress through the program and lab review (see options 1 and 2 below)
• Communication between sessions to provide support as things come up (via emails/chats and calls, as per agreement and availability)
• Coaching assessments and tools using motivational interviewing
 
*Costs for labs and supplements are not included in the program price.
 
International participants: Data will be uploaded manually from your preferred lab to generate the report. (Membership to Apollo Health is required to download lab data for international participants.
 
Participants in the USA: Results from Lab Panel purchased through Apollo Health will be automatically uploaded to generate a report.
